Appointment Information

- All Ages

- Must physically be located in CT at the time of the appointment

- Medical Cannabis patients must be 18 years old to be certified and have a valid state of CT ID

- Payments at booking are non-refundable

- Appointments rescheduled 24 hours in advance will have the fee paid applied to the new appointment

- Appointments that are cancelled within 24 hours or no-showed will forfeit the cost of the appointment paid

- If appointment is made and care is unable to be provided, patient will be referred to appropriate physician for care

- Patients are required to have a patient portal for the practice

- There is no on-call physician available

- Services provided are not meant to be urgent care

- Please call 911 for emergencies or visit your nearest emergency department for immediate care
